Who Should Attend: This initiative will provide an interdisciplinary structure for training, implementation, and even institutionalization of this integrated training model to public health professionals and volunteers including, but not limited to, nurses, diabetes educators, pharmacists, cessation specialists, health care providers, social workers, and paraprofessionals.

Objectives:

Increase the use of the Ask Advise Refer brief intervention in diabetes and tobacco programs
Increase the use of PA cAARds among community based health care organizations
Improve understanding of statewide and local resources for diabetes education and management and tobacco cessation services
Improve understanding of integrated strategies and identify where organizations fit into the organizational integration continuum

Cost: There is no charge to attend this meeting.

Registration Includes: Meals, materials.

Continuing Education Credits: CEU's will be provided at no charge to participants (Nursing, Diabetes Educator, Dieticians, Social Workers, and General)
